Аналіз результатів монокондилярного ендопротезування в умовах зниженої щільності кісткової тканини

Резюме

Актуальність. Біль є основною причиною ендопротезування суглобів, але для багатьох пацієнтів це дає можливість відновити функціональні спроможності. Анкети для самозвітів відображують сприйняття людьми власного стану здоров’я та задоволеність ним. Більше того, фізичні функції краще характеризують самі пацієнти, ніж лікарі. Мета: визначити ефективність розробленої методики монокондилярного ендопротезування колінного суглоба в умовах зниженої щільності кісткової тканини. Матеріали та методи. Були досліджені 3 групи пацієнтів: І група — 59 (67,8 %) осіб, пацієнти з нормальною щільністю кістки, яких оперували за класичною методикою монокондилярного ендопротезування; ІІ група — 6 (6,9 %) осіб, пацієнти зі зниженою щільністю кісткової тканини, яким було проведено монокондилярне ендопротезування за авторською методикою, ІІІ група — 22 (25,3 %) пацієнти зі зниженою щільністю кісткової тканини, яким виконували монокондилярне ендопротезування колінного суглоба за класичною методикою. Усім пацієнтам до та через рік після ендопротезування було проведене анкетування за опитувальником KOOS. Результати. До лікування пацієнти відчувають значні незручності з колінним суглобом, що проявляється больовим синдромом при рухах колінного суглоба — згинанні та розгинанні, ходьбі не тільки сходами, а й по рівній поверхні. Хворі в більшості випадків скаржаться на набряк колінного суглоба, болі вночі та після тривалого відпочинку. Симптоматика більше проявляється в пацієнтів старшого віку (понад 65 років) та у хворих зі зниженою щільністю кісткової тканини — остеопорозом, що є ускладнюючим фактором. Стан пацієнтів після ендопротезування також залежить не тільки від якості проведеної операції, а й від віку пацієнта, тобто чим більший вік хворого, тим вища ймовірність гіршого результату, а також від наявності супутніх захворювань, особливо остеопорозу. Гірші результати ендопротезування у пацієнтів ІІІ групи були пов’язані з ускладненнями, які в деяких хворих призвели до ревізій. Висновки. У пацієнтів ІІ групи, яким було зроблене монокондилярне ендопротезування за новою методикою, результати наближалися до таких І групи — пацієнтів без остеопорозу. З огляду на те, що в ІІ групі були пацієнти доволі похилого віку, результати ендопротезування трохи нижчі, але близькі до таких І групи та кращі, ніж у пацієнтів ІІІ групи. Ускладнень у пацієнтів ІІ групи не було. Авторська методика монокондилярного ендопротезування колінного суглоба дозволяє не тільки отримати хороші результати на фоні остеопоротичних змін, а і збільшити вікову границю оперованих хворих, вона може бути запропонована до більш широкого використання.

Background. Pain is the main cause of joint arthroplasty, but for many patients it allows to restore functional capacity. Self-report questionnaires reflect people’s perceptions and satisfaction with their own health. Moreover, physical functions are better characterized by patients themselves than by physicians. The purpose was to determine the effectiveness of the developed method of monocondylar knee arthroplasty in conditions of low bone density. Materials and methods. 3 groups of patients were studied: Group I — 59 (67.8 %) people, patients with normal bone density, who were operated on by the classical method of monocondylar endoprosthesis; Group II — 6 (6.9 %) patients with low bone density, who underwent monocondylar endoprosthesis according to the author’s method, Group III — 22 (25.3 %), patients with low bone density who underwent monocondylar endoprosthesis knee joint according to the classical method. All patients before and one year after arthroplasty were surveyed according to the KOOS questionnaire. Results. Prior to treatment, patients experience significant discomfort with the knee joint, which is manifested in the presence of pain during movements of the knee joint — flexion and extension, walking not only on the stairs but also on a flat surface. Patients in most cases complain of swelling of the knee joint, pain at night and after a long rest. Symptoms are more pronounced in elderly patients — over 65 years, and in patients with low bone density — osteoporosis, which is a complicating factor. The condition of patients after arthroplasty also depends not only on the quality of the operation, but also on the patient’s age, ie, the older the patient, the greater the likelihood of worse results, as well as the presence of comorbidities, especially osteoporosis. Worse endoprosthesis outcomes in group III patients were due to complications that led to revisions in some patients. Conclusions. In patients of group II, who underwent monocondylar endoprosthesis according to the new method, the results were close to the results of group I — patients without osteoporosis. Due to the fact that in group II there were quite elderly patients, the results of arthroplasty were slightly lower, but close to the results of group I and better than in patients of group III. There were no complications in patients of group II. The author’s technique of monocondylar knee arthroplasty allows to obtain good results not only against the background of osteoporotic changes, but also to increase the age limit of operated patients and can be offered for wider use.
